Use recognized placement criteria, including ASAM criteria when applicable, to identify an appropriate level of support.

Review progress, changing needs, recovery stability, and whether treatment recommendations should be adjusted.

What to expect

A collaborative assessment—not a judgment.

Your clinician will ask about current concerns, behavioral health history, substance use when relevant, relationships, daily functioning, safety, strengths, culture, supports, and goals.

The process may include screening tools, record review, diagnostic evaluation, level-of-care criteria, and coordination with other providers when you authorize it.

Afterward, we review the findings and recommendations with you. Next steps may include outpatient therapy, substance-use counseling, group or family services, recovery support, continuing care, or coordination with another appropriate provider.

Solace Grove provides clinical substance-use and AODA assessments. Wisconsin’s official Intoxicated Driver Program assessment and driver safety plan process is separate: the official assessment must be completed by a county-designated, DHS-approved assessment agency.

What assessments does Solace Grove provide?

We provide mental health, substance use and AODA, co-occurring, diagnostic, and level-of-care assessments based on clinical need and provider qualifications.

Do you provide AODA assessments?

Yes. We provide clinical alcohol and other drug use assessments, treatment recommendations, and outpatient recovery support.

Do you conduct official Wisconsin OWI assessments?

Official Wisconsin Intoxicated Driver Program assessments must be completed by a county-designated, DHS-approved assessment agency. Contact us and we can help clarify the correct pathway and discuss eligible outpatient services following an assessment or referral.

What happens after an assessment?

Your clinician reviews findings, recommendations, and appropriate next steps with you. When another service is indicated, our care team supports coordination and referral.
